Case Manager

About the Role:
As a Family Coordinator, you’re an essential member of our client operations team, responsible for guiding new clients and matched providers through the onboarding process. Your goal is to establish a smooth start to services, secure the right staffing, and set clients up for success before transitioning them to a dedicated Case Manager.

Key Responsibilities:

  • Client Engagement: Build trust with new clients, conducting introductory calls and guiding them through the intake process for a seamless transition to ongoing care.
  • Assessment Coordination: Connect families with their assigned BCBA, follow up on assessments, and coordinate timely submissions to ensure prompt service start.
  • Staffing and Scheduling: Match clients with ideal providers, oversee the first service session, and confirm orientation completion and billing.
  • Performance Monitoring: Regularly check in with providers, address any support needs, and collaborate with Quality Assurance to ensure excellence in care.
  • Documentation and Compliance: Maintain accurate records for all client and provider interactions, ensuring adherence to HIPAA and confidentiality standards.
